Please arrive on time for your appointment. We require 24 hours advanced notice for cancellation. When you arrive at our office you will be given 3 forms to sign. The first is an informed consent that allows us to examine and treat you. The second is our financial policy and the third is a notice of privacy practices and describes how we will protect your private information. You will be given the originals to take home. Thank you, we know our forms are long, but we strive to leave no stone unturned in our quest to help you achieve optimal health.
